Determining Your Best Options in California or Texas
When aging at home is not an option for seniors, an Assisted Living Community setting might be appropriate for them, in order to identify which setting is best for our clients, we perform a comprehensive assessment which includes; Fall Risk Evaluation, Geriatric Depression evaluation, Self-Medication assessment, Activities of Daily Living (ADL’s), Mini Mental State Examination (MMSE), Pain and Skin assessments as needed. Having an independent assessment prior to placing the older adult is important for many reasons:
- Obtain an unbiased assessment.
- Obtain a baseline.
- Compare independent assessment to the assessment done by the community to ensure the correct services are provided and charged accordingly.
Benefits of Working With A Gerontologist & Geriatric Care Manager Call 949-346-5960
Geriatric Care Management Assisted Living Board and Care Crisis Management Memory Care
Assistance from a Geriatric Care Manager in Rowlett, Rockwall or North Texas with ongoing monitoring can prevent costly crises and unnecessary hospitalization. Geriatric Care Managers can be especially helpful when family members live far apart or when there are family dynamics and a facilitator is needed. Sometimes seniors are fearful of losing their independence and they don’t feel comfortable sharing their fears and concerns with their loved one. A Geriatric Care Manager will act as a liaison with families to help maintain good communication with families and other support systems to build or maintain important links between seniors and their families and, with permission, to alert families to problems areas that they may be able to assist with.
Geriatric Care Managers can reduce inappropriate institutional care and overuse of services as well as reduce miscommunications, time, stress, and cost to clients.

Testimony
Yolanda has a heart of gold.
Every ounce of her being is about serving her clients and making sure you get exactly what’s right for you — at least that’s been my experience with her helping me with my 93-year-old mom. Yolanda listened intently to our concerns and needs, she asks great questions, and her advice has always been spot on. Working with Yolanda has made me feel safe and comfortable with some difficult choices, which I deeply appreciate especially when it comes to caring for my precious and vulnerable mom.

Dear Yolanda,
Thank you for the support and comfort you provided during this time of sorrow. I was very close to my dad, and his passing was a shock to my family. It was very comforting to have your expertise, guidance, and support during these difficult times. You truly care for people and it shows.
