Medicare is a federal health insurance program that gives you access to specific coverage and benefits. At The Way To Aging we help seniors find the best Medicare Plan. Beginning October 15th and ending December 7th, the Medicare Annual Enrollment Period (AEP) is generally the time when you can join or switch Medicare Advantage or Part D prescription drug plans.

Our Approach
It is truly a pleasure and an honor for us to serve the elderly population and their loved ones. We understand the importance of treating elders as the whole person by assessing the psychological, physical, social, emotional, and spiritual aspects of aging in order to determine the best approach to create a personalized care plan and enhance the person’s life. Many times they are unique family dynamics that we must work through in order for everyone to be on the same page and focus on the wellbeing of the client, always keeping in mind that each situation is unique and requires a personalized approach.

Giving Your Loved Ones the Care They Deserve
During an in-home or virtual evaluation, we perform a comprehensive assessment which includes; Fall Risk Evaluation, Geriatric Depression evaluation, Self-Medication assessment, Activities of Daily Living (ADL’s), Mini Mental State Examination (MMSE), Pain and Skin assessments as needed. We then create a personalized care plan, and with the family’s input and approval together the care plan is implemented.
We firmly believe that a multidisciplinary team is needed when working with the elderly. The multidisciplinary team shall consist of a Physician, Nurses, Psychologist/Psychiatrist, Geriatric Care Manager, Occupational Therapist, Caregiver, other Healthcare providers (ex: Hospice, Neurologist, Home Health, etc.), spiritual leader, family and the most important member of the multidisciplinary team which is the elder.

Our goal is to provide suggestions, resources, and education to the elderly and their loved ones for them to make decisions that work best for them. When working with people with Dementia, we provide useful behavioral management information and strategies for caregivers and family members to decrease and re-direct challenging behaviors such as aggression and agitation as needed.
